Guarantee Policy All materials and workmanship are fully guaranteed. We do however, might charge 50% of the price for remakes under the following circumstances: · If the case is completed after notification to the doctor of poor impressions or model. · If we request a try-in and doctor says to complete case. · If we have to remake because dentist gives us a new bite or alters preparations. · If we receive no study model for anterior cases and/or no specific instructions are given. · If the shade is different from the original request. If there is a remake, please send us back the original dental restoration, models along with the first and second impression. Billing Policy Customers will receive monthly statements reflecting charges for the current billing cycle. All Accounts are payable in full within 30 days of statement date. Accounts not paid within stated terms will be subject to C.O.D. status and a late charge of 1.5% of the unpaid balance each month (18% APR).   |
